For your convenience the following forms are available to fill out and print in advance of a Chapter Event. This form is required to be signed by all adult and minor members annually and presented with membership dues. This form must be signed by adult guest attending any Chapter Event (Note: only one guest per member allowed). This form must be signed by all Minor's parent or guardian for each Chapter Event whether the Minor is a member or not. This form must be signed by all Minors participating in each Chapter Event whether the Minor is a member or not. When possible please fill out and print these forms and present to the Ride Leader or Event Leader on the day-of-event prior to the Chapter Event start. Other forms |
